Law requires that you provide a form of personal identification on the approved list. By law, no other documents or certificates, such as social security cards, birth certificate, student ID, membership ID, etc. may be accepted.

Names on documents being notarized must match what is listed on the accepted form of identification. You may need to request adjustments be made to your documents or verify that you may attach a notarial certificate with the name that matches your ID.
